F4CP to host webinar on medical home model

The Patient-Centered Medical Home (PCMH) is becoming a dominant model for healthcare delivery in your community — and throughout the U.S.

Presently, the public and private sectors have implemented some 27 multi-stakeholder pilot programs in 18 states, resulting in 14,000 certified PCMH cites nationwide, with many more set to launch later this year.

While chiropractors are recognized as physicians under the Joint Commission, and considered to be eligible members of PCMH care teams under the Patient Protection and Affordable Care Act, there is no mandate to include them.

You are encouraged to ensure that you are part of this next phase in healthcare delivery and included within your community’s PCMH. To assist with opening the doors to this exceptional opportunity, the Foundation for Chiropractic Progress (F4CP) has developed a white paper to serve as guidance.

On Oct. 4, 2011 at 1 p.m. EST, the foundation will host a webinar on its newest white paper, “The Role of Chiropractic Care in the Patient-Centered Medical Home.”  The presentation will feature a dynamic panel of experts, including: Laura Carabello, Dr. Gerard Clum, Dr. Michelle Maiers and Guy D’Andrea, who will discuss the PCMH, articulate the position of a DC within the model and provide guidance regarding local integration.
